Medical Forms
 All Medical Forms must be submitted by June 1st. No camper will be allowed to depart for camp unless we have received his or her Medical Forms.

Please make your appointments now so that you can complete the forms by June 1st.

All Health Forms are completed through the forms section of your CampInTouch account.

CampMeds is a pre-packing medication program that dispenses all camper medications directly to our camp Health Center.

Timber Lake families must send ALL MEDICATIONS to camp through Camp Meds. The ONLY EXCEPTIONS are: over-the-counter vitamins, creams, ointments, Accutane, growth hormones, insulin, EpiPens, injections, Lactaid pills, and birth control.

These items should be packed in your child’s duffel, clearly labeled and MUST be documented on their Camper Physician Form. If an item requires REFRIGERATION, it should be labeled with your child’s name and given to the camp supervisor at your respective bus stop.

To REGISTER with CampMeds, visit for instructions. CampMeds must receive all information outlined in their sign-up sheet by May 22nd to avoid any late fees. Note: If your child does not take any medications, you do not need to register with CampMeds.

Our Health Center stocks many commonly used over-the-counter items such as Tylenol, Advil, Benadryl, Zyrtec, Robitussin, Dramamine, Mylanta and Tums. Therefore, there is NO NEED to have CampMeds dispense typical over-the-counter items. However, if your child takes any of these items on a DAILY basis, it must be dispensed through CampMeds.

As a reminder, your child’s physician is required to complete the Over-the-Counter Medication form in the “Forms & Documents” section of your CampInTouch account.

New York State requires a full listing of up-to-date physician-documented immunizations (including current tetanus).
